Veterans In Partnership Healthcare Network (VA VISN 11) to Deploy Avicenna's Medication Reconciliation Tool

Avicenna Medical Systems, a medical software developer, is proud to announce that The Department of Veterans Affairs Veterans In Partnership Healthcare Network (VA VISN 11), will be deploying Avicenna’s medication reconciliation solution, MedRec Tool, across both inpatient and outpatient VA health care facilities in central Illinois, Indiana, Michigan and northwest Ohio.

Ensuring that a patient’s medications are accurate is a crucial step to avoid medical errors and optimize care. For many busy caregivers, the process of verifying a patient’s medication history can be cumbersome and time-consuming. With Avicenna’s MedRec Tool, VA caregivers and clinical pharmacists will now have the patient’s complete medication history within easy reach. MedRec Tool’s unique architecture easily integrates with the Veteran Health Information Systems Technology and Architecture (VistA) and its Computerized Patient Record System (CPRS), combining VA pharmacy and VistA/CPRS data with a user-friendly interface that improves, not impedes caregiver workflow. MedRec Tool’s versatility can be customized to meets the VA’s unique needs in both inpatient and ambulatory clinical settings across multiple healthcare locations. MedRec Tool is the only known tool that aggregates medications from multiple VA care centers, non-VA sites, and remote medications found in the VA’s VistA Web database. MedRec Tool also provides patients with an easy to understand medication summary outlining new, changed, or discontinued medications.

Veterans In Partnership Healthcare Network (VISN 11) is one of 21 Veterans Integrated Service Networks (VISN) of the Department of Veterans Affairs. VISN 11 is comprised of seven VA Medical Centers and twenty nine operating Community Based Outpatient Clinics (CBOC) which provide comprehensive inpatient and outpatient health care to veterans in central Illinois, Indiana, Michigan and northwest Ohio. VISN 11 is a large and geographically diverse network and covers 90,100 square miles. The VIP network medical centers operate 1812 beds including acute medical and surgical, psychiatric, and nursing home beds. 

VISN 11 takes pride in the fact that as an organization it provides the best possible care to veterans within its service area. In fiscal year 2009, VISN 11 provided care to 248,935 unique Veteran patients 

that translate to 27,092 inpatient stays and 218,442 bed days of care and 2,462,616 outpatient visits.
