Hypertension awareness, treatment and control remain persistent challenges in the U.S., according to an article published in the American Journal of Hypertension. That is where telehealth can help.

The measure set, proposed by the Physicians Foundation, was adopted for use earlier this month in the Hospital Inpatient Prospective Payment System rule, effective 2023. These measures are based on the foundation’s framework for addressing SDOH (PDF), published last year. 

Priority Health's Medicare Advantage members in 14 Michigan counties will gain access to Homeward's in-person, virtual, and remote monitoring services through the partnership.

The Innovation Center has tested models to increase access to advanced, integrated primary care in many communities. It reviewed data and practice feedback from several primary care initiatives:  the Comprehensive Primary Care (CPC) and Comprehensive Primary Care Plus (CPC+) models and the Primary Care First (PCF) model. Five major lessons emerged from this review.
